Direct Support Professional / Community Connections Day Program

Position Overview

United Cerebral Palsy of Hudson County (UCP) is seeking a compassionate and reliable Direct Support Professional (DSP) to join our Community Connections Day Program. In this role, you will provide hands-on personal care, facilitate skill-building activities, and support consumers in both home and community environments.

The ideal candidate is patient, dependable, and passionate about helping individuals with intellectual and developmental disabilities (IDD) live more independent and fulfilling lives.

Key Responsibilities

Direct Care & Support

  • Assist consumers with daily living tasks including toileting, feeding, grooming, and hygiene
  • Maintain consumer dignity and respect at all times
  • Provide safe and age-appropriate communication and interaction
  • Support transfers and mobility using proper lifting techniques and adaptive equipment
  • Encourage participation in social, recreational, and educational activities based on each individual’s preferences and goals

Community Integration & Skill Development

  • Implement individualized support plans (ISPs)
  • Teach and reinforce daily living skills both in-house and in the community
  • Promote independence through role modeling and prompting appropriate behaviors
  • Support consumers in achieving ISP objectives through structured activities

Transportation Support

  • Assist in safely loading/unloading consumers onto UCP-owned vans
  • Secure individuals and wheelchairs with proper restraints
  • Follow all transportation policies and emergency procedures

Documentation & Communication

  • Accurately record care provided and progress toward ISP goals
  • Use Electronic Health Records (EHR) per agency protocols
  • Maintain HIPAA compliance and consumer confidentiality
  • Participate in interdisciplinary team meetings and contribute to program planning

Qualifications

  • High School Diploma or GED required
  • 1 year of experience working with individuals with IDD preferred
  • Must be at least 18 years of age
  • Strong communication skills and ability to work in a team environment
  • Compassionate, respectful, and professional demeanor

Certifications Required

  • Current CPR Level C Certification
  • Current First Aid Certification

Physical Requirements

  • Frequent walking, standing, and lifting (25+ lbs)
  • Regular contact with consumers, including lifting, repositioning, and transporting
  • Frequent van transportation responsibilities
  • Exposure to odors, bodily fluids, and infectious diseases
  • Ability to perform two-person lifts and utilize adaptive equipment safely

Driver / Community Connections Day Program

Position Overview

United Cerebral Palsy of Hudson County (UCP) is seeking a dedicated and responsible Driver to transport our consumers to and from a variety of destinations using UCP-owned vehicles. You will play a vital role in ensuring safe and reliable transportation while supporting individuals with disabilities in their daily activities and community integration.

This role includes driving, basic vehicle upkeep, and assisting consumers with personal care and activities of daily living as needed.

Key Responsibilities

Transportation Duties

  • Transport consumers to day programs, medical appointments, educational and recreational activities
  • Follow scheduled routes and adjust for additional trips as needed (including overtime)
  • Maintain cleanliness and basic maintenance of UCP vehicles
  • Ensure vehicles are inspected and compliant with UCP and state requirements
  • Complete daily vehicle checklists and report any issues promptly

Direct Care Support

  • Assist consumers with meals, toileting, and personal hygiene
  • Support transfers and use of adaptive equipment
  • Help consumers achieve personal goals through community activities and skill development
  • Communicate effectively with consumers using age- and ability-appropriate methods

Safety & Compliance

  • Perform all duties in line with UCP safety protocols and standards
  • Maintain stocked first aid kits and report health/safety concerns
  • Ensure confidentiality and respectful communication with all consumers

Qualifications

  • High School Diploma or GED preferred
  • Must be 18 years or older
  • Valid New Jersey Driver’s License required
  • Commercial Driver’s License (CDL) preferred (if applicable)
  • Must provide and maintain a clean Motor Vehicle Record (MVR)

Certifications Required

  • Current CPR Level C Certification
  • Current First Aid Certification

Physical Requirements

  • Exposure to varying weather and environmental conditions
  • Frequent operation of motor vehicles and loading/unloading of consumers

Residential Program Developer

About the Role

UCP of Hudson County is seeking an experienced and mission-driven Residential Program Developer to lead the design, implementation, and oversight of residential services for individuals with intellectual and developmental disabilities (I/DD). This position will play a critical role in developing new residential programs, ensuring regulatory compliance, and creating high-quality, person-centered supports that promote independence, safety, dignity, and community inclusion. The ideal candidate is someone who can build programs from the ground up while also providing hands-on operational leadership.

Key Responsibilities

Program Development & Launch

· Design and implement residential service models, daily programming, treatment supports, and operational systems.
· Assist with the development and startup of new residential programs, including group homes, supportive housing, and community-based living arrangements.
· Develop policies, procedures, staffing plans, and service delivery models.
· Create and maintain individualized supports and programming based on participant needs and goals.
· Promote resident independence, life skills development, and meaningful community engagement.

Regulatory Compliance & Quality Assurance

· Ensure compliance with all applicable federal, state, Medicaid, licensing, and agency requirements.
· Prepare programs for inspections, audits, and licensing reviews.
· Coordinate with regulatory agencies, healthcare providers, and community partners.
· Maintain accurate records, incident reporting, quality assurance measures, and documentation standards.
· Monitor program performance and implement continuous quality improvement initiatives.

Leadership & Staff Development

· Recruit, hire, train, supervise, and evaluate residential staff, including Direct Support Professionals (DSPs) and other support personnel.
· Provide coaching, mentoring, and performance management.
· Ensure staff receive required training in participant care, behavioral supports, health and safety practices, and agency policies.
· Foster a culture of accountability, professionalism, and person-centered service delivery.

Participant Support & Care Coordination

· Collaborate with families, guardians, clinicians, support coordinators, and interdisciplinary teams.
· Develop and monitor Individualized Service Plans (ISPs), behavioral support plans, and person-centered goals.
· Support participants in achieving maximum independence and quality of life.
· Coordinate recreational, social, vocational, and life-skills opportunities.

Operations & Program Management

· Oversee daily residential operations, including staffing coverage, scheduling, transportation coordination, purchasing, household management, and site maintenance.
· Monitor budgets and expenditures to ensure fiscal responsibility.
· Maintain safe, structured, and supportive living environments.

Crisis Response & On-Call Support

· Respond to participant emergencies, incidents, and crisis situations.
· Provide guidance and support to staff during emergencies.
· Participate in an on-call rotation and provide after-hours support as needed.

Family & Community Relations

· Serve as a liaison between residents, families, guardians, healthcare providers, state agencies, and community partners.
· Maintain professional, compassionate, and effective communication regarding services and participant progress.

Qualifications

Required

· Bachelor’s degree in Social Work, Psychology, Human Services, Healthcare Administration, or a related field; equivalent experience may be considered.
· Minimum 3-5 years of experience in residential services, developmental disabilities, behavioral health, human services, or a related field.
· Experience working with individuals with intellectual and developmental disabilities.
· Strong knowledge of residential program operations, staffing, compliance standards, licensing requirements, and person-centered services.
· Excellent leadership, organizational, problem-solving, and communication skills.
